Service Description

Provides workshops to help individuals with chronic conditions take charge of their life and find practical ways to deal with chronic conditions. As well as training for persons interested in becoming a ‘Peer Leader’ to co-lead the six-week program.

  • Groups meet for two and a half hours a week for six weeks.
  • For individuals who have a chronic condition, including (but not limited to):
    • Anxiety/Panic Disorder
    • Arthritis
    • Asthma/COPD/Lung disease
    • Cancer
    • Chronic Heartburn/Acid Reflux
    • Chronic Pain
    • CHF
    • Depression
    • Diabetes
    • Hepatitis/HIV
    • High Blood Pressure
    • Inflammatory Bowel Disease
    • Kidney Stones
    • Multiple Sclerosis
    • Parkinson’s
    • Renal Failure
    • Stroke

Participants:

  • Receive information on problem-solving and action plans, exercise, cognitive symptom management, nutrition, fatigue and symptom management, use of medications, dealing with difficult emotions, communications and more.
  • Set their own goals and make a step-by-step plan to improve their health and quality of life.
  • Receive a free copy of the book Living a Healthy Life with Chronic Conditions as well as Living a Healthy Life with Chronic Pain and Moving Easy program CD.

For individuals who have a chronic condition, including (but not limited to):
