Service Description

Provides diabetes education, care and support to persons with pre-diabetes (IFG/IGT), diabetes (Type 1 & Type 2),  gestational diabetes (GDM), hypoglycemia, or polycystic ovarian syndrome (PCOS).

  • Self-management skills taught include: healthy eating, active living, glucose monitoring, medications, and other lifestyle issues such as travel, restaurant eating and shift work.
  • MEDEC educators provide the training and skills needed to start insulin therapy, adjust dosage, and to use an insulin pump.

Endocrinologist (Diabetes Specialists) Services

  • Provides medical assessment and specialized care for adults with all types of diabetes.

Pregnancy and Diabetes Clinic

  • Provides specialized, comprehensive diabetes management and prenatal care for women with type 1 and type 2 diabetes during pregnancy, and for women with gestational diabetes, who require insulin therapy.

Rural Diabetes Education Programs

  • Offers education to adults with pre-diabetes, type 2 diabetes, or gestational diabetes (GDM), in rural areas within the health region.

Eligibility

Adults, youth and children with type 1 and type 2 diabetes, pre-diabetes (IFG/IGT), gestational diabetes (GDM), hypoglycemia, or polycystic ovarian syndrome (PCOS).

Application Process

Self-referrals and referrals from physicians and other health professionals are accepted. A health card is required.
