Service Description
Provides a record of immunization for clients, physicians, and public health offices within 7 days of client request.
Eligibility
Individuals who have lived or attended school within Saskatoon Health Region. Contact for further details.
Application Process
Call 306-655-4090: The Record Search Line is staffed Monday - Friday. When a call is not answered by a live operator, leave a message including your first and last name, date of birth, Saskatchewan Health Card, day/time, and phone number; staff will respond to your call within five business days.
-Immunization Record Request Form: Fill out the form and fax (306-655-4711), or mail (Population & Public Health
North East Office, Immunization Record Request, #108 – 407 Ludlow Street, Saskatoon, SK, S7S 1P33) it to the Record Search staff.
Fees
Free for children/youth under 16 years of age
$25 - Individuals over 16 years of age

Child Health Clinic: call to make an appointment or Drop-in (for families who live in and around the neighbourhood): third Thursday of the month 1:00 - 7:00 pm.
Flouride Varnish 2nd Tuesday of the month, 9:00 - 11:30 am
Adult Immunization: call to make an appointment; Drop-in: fourth Thursday of the month 12:00 - 3:00 pm, 3:30 - 7:00 pm.
