Service Description
Groups of up to 12 participants meet for 2.5 hours once a week for six consecutive weeks. Emphasis is on building skills for managing your health and maintaining an active and fulfilling life. Assists you to take control of your chronic condition rather than the illness controlling you. Self-management skills in the program include: symptom management, communication skills, relaxation techniques, goal setting, and problem solving.
Eligibility
Caregivers and/or individuals who have a chronic health condition such as: Chronic Pain, Arthritis, Diabetes, Heart Disease, Chronic Lung Disease, Parkinson’s Disease, Stroke/Spinal Cord Injury, Osteoporosis, Multiple Sclerosis, Kidney Disease.

Participants receive a free copy of the companion book, Living a Healthy Life with Chronic Conditions.
