Service Description

Educates and supports individuals who are at risk for heart disease, or who are already affected by heart disease, to identify and manage their risk factors.
Program also available by telehealth in Cabri, Leader, Maple Creek, Ponteix, and Shaunavon.

Eligibility

Individuals who are at risk for heart disease, or who are already affected by heart disease.

Application Process

Call 306-778-5118 to register.
* If this program is wanted via Telehealth in Cabri, Leader, Maple Creek, Ponteix, and Shaunavon, must be requested at time of inital call as it requires advance set-up.
