Service Description
Offers comprehensive clinical practice for individuals with diabetes, including:
- Individual appointments with clients to assist them in attaining the skills needed to live healthy with diabetes
- A foot clinic
- A one-day multi-disciplinary “Survival Skills” diabetes education class
- Initiate and adjust insulin to assist clients in achieving optimal glycemic control
- Assistance for pump clients to initiate and maintain insulin pump therapy
- Dietitian counseling for any other outpatient dietary referrals.
Eligibility
Individuals who are pre-diabetes, Type 1 Diabetes, Type 2 Diabetes, Gestational Diabetes and pregnancy with pre-existing Diabetes.
