Service Description

Offers comprehensive clinical practice for individuals with diabetes, including:

  • Individual appointments with clients to assist them in attaining the skills needed to live healthy with diabetes
  • A foot clinic
  • A one-day multi-disciplinary “Survival Skills” diabetes education class
  • Initiate and adjust insulin to assist clients in achieving optimal glycemic control
  • Assistance for  pump clients to initiate and maintain insulin pump therapy
  • Dietitian counseling for any other outpatient dietary referrals.

Eligibility

Individuals who are pre-diabetes, Type 1 Diabetes, Type 2 Diabetes, Gestational Diabetes and pregnancy with pre-existing Diabetes.
