Service Description

Individual Assessments and Follow-Up Counselling Sessions

    • Provides assessment of needs, teaching, counselling and goal-setting support

      Diabetes / Prediabetes Class Program

      • A series of four, 90 minute classes offered on a weekly basis. Spouses, family members, or other support person(s) are welcome to attend classes. Classes are offered at the Patient Education Centre in Moose Jaw and by TeleHealth at the other sites.

      Gestational and Pregnancy Diabetes Program

      • Offered for patients with diabetes during their pregnancies

      People living with diabetes or prediabetes.
