Service Description

Provides information on and vaccines for routine infant, pre-school and school immunization programs that protect residents against disease, as well as those vaccines for targeted programs for persons at high risk.  An immunization schedule and information is posted on the website.

  • To obtain a copy of your (or your child’s) immunization records, visit the website and complete the Access Form.
