Service Description
Provides treatment, health promotion, and rehabilitative health services for band members living on reserve of any age to assist them in staying in their own home. Services are based on a needs assessment. Examples include teaching how to provide care such as needles, wound care, medications and help with bathing, foot care, or dressing.
Eligibility
Members of the Saskatoon Tribal Council including Kinistin Saulteaux, Mistawasis Nêhiyawak, Muskeg Lake, Muskoday, One Arrow, Whitecap Dakota, and Yellow Quill First Nations who meet one of the following criteria:
- Just came home from the hospital
- Are waiting for care in a long-term care home
- Have an illness or disease that requires follow-up
- Need nursing care in the home
- Choose to live at home instead of a long-term care home as long as it is safe
- Have an assessed need or disability requiring assistance to live independently
- Have family and friends who need help to continue to provide care at home
