Service Description
A five-session education and support program of the Heart and Stroke Foundation of Canada that assists cardiac patients and their families who have, or are at high risk for cardiovascular disease.
Eligibility
Cardiac patients and their families who have, or are at high risk for, cardiovascular disease. Spouses, family members, friends, or other support persons are welcome to attend as well.
