Service Description

Provides a monthly taxable benefit to contributors who are disabled and to their dependent children.

Eligibility

Persons 64 years of age or younger. Stopped working because of medical condition. Have paid into the CPP for at least four of the last six years or have paid into the CPP for at least 25 years and made valid contributions to the Plan in three of the last six years.

Application Process

Available online. Must be made within 15 months of the start of the persons CPP retirement pension. Also, any CPP retirement pension payments already received may be deducted from the persons disability benefit.

Disability Benefits Form. Questionnaire for Disability Benefits Form. The Medical Report Form. Consent for Service Canada to obtain personal information. The Child Rearing Provision Form.
