Secure computer system containing information about resident’s prescribed and dispensed medication accessed confidentially by authorized health care professionals, details here
- patient can request copy of, or an amendment to, their medication profile
- patient can request to mask or unmask medication profile

No restrictions
Application Process
Request copy of medication profile from an authorized health care provider (e.g. pharmacist) or the Ministry of Health
Request to mask personal information by completing form, or call eHealth Privacy Service
Remove mask by completing this form
Request amendment by writing to the Ministry of Health, or calling Drug Plan and Extended Benefits Branch
