Service Description

Provides nursing assessments and counselling, establishes and implements treatment plans for individuals, and provides education support, and advocacy. A community partnership model is used that may include third party arrangements with independent community services in treatment plans.

Eligibility

Persons with long-term psychiatric disabilities. Long-term clients are those individuals who have a mental disorder which has resulted in hospitalization or a significant loss of social functioning for over 18 months including those who have been acutely ill or traumatized and are at risk of developing long term disabilities.

Application Process

Referrals to the program may be by individuals themselves, physicians, and court mandated professionals. Contact the Regional Centralized Intake. The program prioritizes services to the long-term mentally ill person and to their families on a case-by-case basis based on risk, impairment, distress, urgency, availability of support and mandate.
