Service Description
A 6-week self-management workshop for persons living with a chronic condition and/or their caregivers, to build skills for managing your health and maintaining an active and fulfilling life.
Course content includes:
- Symptom management (relaxation techniques, self-talk, visualization)
- Goal setting and problem solving
- Nutrition and exercise guidelines
- Communication skills
- How to access community resources
- Managing emotions such as frustration and fear
- How to get the most out of life
Eligibility
Anyone who is experiencing a chronic health condition and their caregivers.
