Service Description

Provides an exercise and education program for people living with or at risk for chronic conditions monitored by health professionals with a self-care management focus. Includes education, exercised, and self-management support.

Eligibility

People living with or at risk for chronic conditions such as (but not limited to): heart disease, lung disease/COPD/asthma, diabetes, arthritis, or stroke.

Application Process

Call the location most convenient to you or ask your health care provider for a referral.
