Service Description

Provides continued therapy for people who have suffered a stroke and need continued therapy. The rehabilitation team consists of a physiotherapist, occupational therapist, speech language therapist, acquired brain injury coordinator, chronic disease nurse, and others.

Eligibility

Individuals who have suffered a stroke and need continued therapy.

Application Process

Call the location most convenient to you or ask your health care provider for a referral.
