Service Description

Education for people living with the following chronic conditions:

  • Diabetes
  • Stroke
  • Heart Disease
  • COPD
  • Asthma.

Eligibility

Individuals living with the following chronic conditions: diabetes, stroke, heart disease, COPD, or asthma.

Application Process

Self-refer or referral from physician/health care professional.

No cost.
