Service Description
An education and support program for cardiac patients and their families.
Eligibility
Individuals (and their family members) who have had a heart attack, heart surgery, angina, or are at high risk for heart disease.
Application Process
Self-refer or referral from physician/health care professional. Call the location most convenient for you or ask your health care provider for a referral.
