Service Description
Offers practical suggestions and support which builds confidence in coping with the everyday challenges of living with a chronic condition.
- Two trained leaders meet with groups of up to 12 participants for two hours once a week for six weeks.
Eligibility
Caregivers and/or individuals who have a chronic health condition such as:
- Chronic Pain
- Arthritis
- Diabetes
- Fibromyalgia
- Multiple Sclerosis
- Heart Disease
- Chronic Lung Disease
- Parkinson's Disease
- Cancer.
