Service Description
Physical and occupational therapists provide assessment and treatment to help maintain or improve the current level of function and to maximize independence for clients, those living in private residence or personal care homes:
- neurological
- post-surgical
- acute ot chronic respiratory
- general mobility issues
- cognitive testing
- assisting with functional assessments
Application Process
Referrals (form is posted on the website) can be accepted from the following: physicians, other physical or occupational therapists or speech language pathologists, nurse practitioners, dentists, chiropractors, podiatrists, social workers, psychologists, community care coordinators, self referral/ family members/ caregivers.
