Application Process

Request copy of medication profile from an authorized health care provider or the Ministry of Health
Request to mask personal information by completing form, or call eHealth Privacy Service
Remove mask by completing form
Request amendment by writing to the Ministry of Health, or by calling Drug Plan and Extended Benefits Branch
