Service Description

A program for caregivers and/or individuals who have one or more chronic health conditions such as (but not limited to):

  • Cancer
  • Chronic Pain Arthritis
  • Diabetes
  • Heart Disease
  • Chronic Lung Disease
  • Parkinson’s Disease
  • Stroke/Spinal Cord Injury
  • Osteoporosis
  • Multiple Sclerosis
  • Kidney Disease.

Included in the 6-week program (2.5 hours weekly) are:

  • Problem solving and action plan
  • Exercise
  • Cognitive symptom management
  • Nutrition
  • Fatigue and symptom management
  • Advanced care directives
  • Use of medications
  • Dealing with fear, anger and depression
  • Communication with others

Helps participants obtain information, learn new skills and abilities, and develop higher levels of confidence to manage and cope with chronic health conditions.  Program is offered on interest.

Caregivers and/or individuals who have one or more chronic health conditions.
