Service Description
Provides services to address the unmet needs of people with cognitive impairment. Assists clients with developing an integrated caseplan. For families, it may mean respite care or additional therapy services for their child.
Eligibility
Open for any age.
Application Process
Call for more information or visit the website for a referral form. Accepts referrals from anyone involved in the care of the child/teenager.
