Service Description

Provides post-adoption contact information for adoptions that occurred in Saskatchewan to adult adoptees, adoptive parents of an adoptee who is under 18 years of age, birth parents, adult children of a deceased adult adoptee, adult children of a deceased birth parent, and extended family members.

Note: Birth parents and adult adoptees are able to place a veto, which prevents the release of their identifying informationon the birth registration. Birth parents and adult adoptees may file a contact preference which allows them to place limits on contact from the other person. The release of identifying information will be subject to any vetoes or contact preferences placed by a birth parent or adult adoptee.

Eligibility

You may be eligible for services if the adoption was finalized in Saskatchewan and you are: - an adult adoptee (18+ years of age) - an adoptive parent of an adoptee who is under 18 years of age - a birth parent of an adoptee - the adult child of a deceased adult adoptee - the adult child of a deceased birth parent whose child was placed for adoption; or - an extended family member of an adult adoptee or birth parent.

Application Process

Gather the documents required for your service application (details available online); complete and print the appropriate service application form online; and submit the completed application form via mail, fax, or email.
