Service Description
Provides a program for children birth – 18 years of age.
- identifying children with unmet needs and assisting the families to secure access to needed services and supports
- ensuring children receive quality and culturally appropriate health and social services
- networking and making connections for families with children with special needs
- attending appointments with families, if requested
- setting up cultural supports for families
- arranging transportation to and from appointments
