Service Description
A multidisciplinary team including a Nurse Practitioner, Traditional Knowledge Keeper, Primary Care Chronic Disease Nurse, Dietitian, Diabetes Nurse Educator, Health Navigator, and pharmacist provide chronic disease management, outreach services, early screening and detection for kidney disease, satellite hemodialysis, independent hemodialysis, and support for home dialysis. Services include:
- Primary care services for conditions such as diabetes, high blood pressure, kidney disease, coronary artery disease, and cholesterol management,
- Individual/family/group health education and counseling,
- Dietitian and Diabetes educator services,
- Tele-health support,
- Visiting specialists,
- Outreach services and health clinics to surrounding communities,
- 24 hour blood pressure monitoring,
- Early detection and screening for kidney disease,
- Outpatient procedures,
- Satellite dialysis,
- Independent dialysis suite,
- Traditional medicine,
- Traditional and spiritual institutes,
- Spiritual counseling,
- Collaboration and referral with other health care professionals.
Eligibility
Clients may be referred by their primary care provider, other health care workers, or make their own appointment.
