Service Description
Program:
- Helps clients take control of their Chronic Obstructive Pulmonary Disease (COPD)
- Enhances quality of life and helps manage symtpoms
- Assists in preventing COPD symptoms and complications
- Works with clients’ doctors to design and implement an individualized plan of action
Education and a supervised exercise programs are offered.
Eligibility
Individuals with a diagnosis of COPD and their family/support person.
Application Process
Referrals to the program are accepted from physicians, specialists, and other health care providers. If you think the program might be right for you, speak with your health care provider.
