Service Description
Provides the following Cardiac Rehabilitation programs:
Heart to Heart is an education program of the Heart and Stroke Foundation for cardiac patients and their families. This is a six week program with classes once a week to help patients gain knowledge of heart disease and cardiac rehabilitation.
Cardiaction Exercise Program is medically supervised and provides a safe, structured program for patients to improve their exercise tolerance and increase their cardiovascular fitness. It is offered three times a week on an ongoing basis and partners are welcome to join.
Support Programs include a Cardiaction Support Group of current and past participants, a Graduate Exercise Program.

Note: Individuals not able to attend the Cardiac Rehab Exercise Program in Moose Jaw, may be assessed and given a home exercise prescription and guidelines.
Eligibility
Individuals and their families after a heart attack, heart surgery, diagnosis of angina, arrhythmias, heart failure or peripheral vascular disease.
Application Process
Referral from doctor or nurse practioner is needed.
