Provides a formal way for patients to address questions or issues with their health care that could not be resolved directly with their health team, including information about
Eligibility
Individuals who have received care or services and have concerns about them
Application Process
When possible, discuss concern directly with health care provider involved or their supervisor
If concern not resolved, select and contact appropriate channel
