Service Description
Supports children/young adults with physical, intellectual, medical, and cognitive disability (as well as their families) to access/create support and services to meet their needs.
- Also works together with individuals and their families through transition to the adult service delivery system.
- Works with other ministries, community groups, and service providers to ensure supports are inclusive and meet the needs identified by the child/young adult and their family.
- Administered by the Heartland Health Region with support and funding from the Ministry of Social Services Community Living Service Delivery.
Eligibility
Families having youth or dependent adults with a physical, intellectual, or cognitive disability.
Application Process
Family can self-refer or other agencies can refer a family with consent. Call for information on how to obtain a form to request support.
