Service Description
Care within the home and community which may include:
- Nurses providing assessment, direct care, education for the client and/or family on self care, medication monitoring, personal care supervision, ordering of equipment and supplies, and referrals.
- Home Health Aides providing personal care, home management, and respite services.
- Supportive Services that are dependent based on availability within the community.
Eligibility
Members of the Touchwood Agency Tribal Council including Muskowekwan, Day Star, George Gordon, and Kawacatoose First Nations who:
- Have come home from hospital
- Are waiting for Long Term Care
- Have an illness or disease that needs follow-up
- Need nursing care at home
- Choose to live at home instead of long term care for as long as it is safe
- Family and friends who need help in continuing to care for a person at home.
