Service Description
Provides support services to First Nations clients who seek services in the Prairie North Health Region. The Hospital Liaison Worker provides the following services:
- Advocacy
- Discharge Planning
- Assistance with Forms
- Referral
- Attending Discharge Rounds
- Supportive Services
- Translation
- Medical Transportation Assistance
- Follow Up
- Reassurance Visits
- Long Term Care Visits
This program is a partnership between the Prairie North Regional Health Authority and the Battle River Treaty 6 Health Centre.
