Service Description

Addresses community access and rehabilitation support. 

Participants are matched with a qualified Community Support Worker to help facilitate each individual’s unique goals and interest. It promotes the active participation of individuals with acquired brain injury in the community. Services include:

  • Provides assistance with independent living, community connections such as volunteer opportunities, and leisure activities that encourage social interaction.
