Service Description
Offers a six-week chronic disease management program to help participants cope with chronic health conditions. Services include:
- Problem solving and action plans
- Exercise
- Cognitive symptom management
- Nutrition
- Fatigue and symptom management
- Advanced care directives
- Use of medications
- Dealing with fear, anger and depression
- Communication with others
This program is delivered under license from Stanford University by volunteer lay leaders.
Eligibility
Caregivers and/or individuals with chronic health condition(s) such as chronic pain arthritis, diabetes, heart disease, chronic lung disease, Parkinson’s disease, stroke/spinal cord injury, osteoporosis, multiple sclerosis, and kidney disease.
