Service Description
Provides services and supports to help reduce use of acute care resources and maintain housing placements for those who have difficulty maintaining their household, managing finances, medication, hygiene, or personal health. Collaborates with existing resources and community supports related to crisis intervention and crisis response. Services are based on needs assessment, individual goals, and wellness plans. Individuals are assigned an intensive case manager and team who will provide assistance 12 – 14 hours a day, seven days a week. (8 – 10 hours on the weekends) Capacity for 20 – 25 individuals.
Eligibility
Persons with serious and persistent mental illness who experience difficulty in maintaining their household, managing finances, medication, hygiene, and personal health.

Referrals made by Community Recovery Services at the Regina Mental Health Clinic. Call 306-766-7800 to speak to intake and receive further information.
