Service Description
Provides home care services to help individuals with health problems stay in their own homes on-reserve for as long as possible. Services are based on each client’s health needs and may include:
- Assessment
- Home Nursing
- Home Health Aide Services
- Dietitian
- Wellness Support Nurse
- Hospital Liaison
- Diabetes Nurse Educator
- Exercise Therapist.
Eligibility
- Individuals who live on-reserve and have health problems that get in the way of their everyday activities.
- Individuals whose family are not able to provide the needed help.
