Service Description
Improves the quality of living and dying by addressing the physical, emotional, psychological, spiritual and practical support of individuals and their loved ones coping with a life threatening illness. The focus of palliative care is to neither hasten nor postpone death but rather to make the last stages of life comfortable and peaceful. Program goals are to prevent and relieve suffering (physical, emotional, social and spiritual). Services provided are:
- Assessment and management of pain and other symptoms
- Physical, emotional, social and spiritual support
- Educational support
- Bereavement – offers a variety of care to help understand anticipate loss and grief
Eligibility
Clients, any age, with a progressive, life threatening incurable illness, no longer seeking a cure, illness is deemed incurable, life expectancy is less than 6 months.
Application Process
Referrals made for service may be initiated by any person directly involved with the client and has the client’s consent. You do not need a physician referral to receive services. Referral may be initiated by contacting your nearest Home Care Office.
Fees
Palliative care services provided in the home will be consistent with Community Home Care charges. Contact your nearest Home Care office for specifics.
