Service Description

Provides clients having heart disease or having a high risk for heart disease with the information they need to help take control of their condition and live a heart-healthy lifestyle. Helps clients and their families with a better understanding of their heart condition, risk reduction for future heart related complications, weight loss/management, controlled cholesterol levels, improved physical fitness, reduced stress, and improved energy levels.

Eligibility

Persons who have experienced heart attack, angina, bypass surgery, angioplasty, or other heart-related conditions and surgeries. The program can also benefit persons who are at high risk of heart disease, such as those with high cholesterol, or have a family history of heart disease.

Application Process

Program is offered to all clients and patients of the Sun Country Health Region. Some lab information is needed - it is best to see your physician to be referred to the Cardiac Rehabilitation Program. Even so, you can still contact the Cardiac Rehabilitation Coordinator directly to join the program - but you may be required to go back to your physician for lab work.
