Service Description

Provides diabetes services for patients and their families to support them in managing diabetes, including:

  • Provides education and self-management support for patients with diabetes
  • Wellness clinics – assess blood pressure, blood glucose levels, weight, oxygen saturations and provide foot care. Dietitians participate in some Wellness Clinics
  • Individual counselling provided by a dietitian, nurse practitioner or diabetes nurse educator.
