Service Description

Provides support to residents living in a home of their choice in the community. Capacity for 15 – 20 individuals. Includes:

  • 24 hour on-call support
  • support provided based on needs assessment, which may include skills of daily living, medication management, financial trusteeship, leisure/social activities, emotional support, etc.
  • a range of services from outreach (minimal) to moderate levels of support.

Eligibility

Saskatchewan residents with an acquired brain injury.
