Service Description
Provides a holistic care approach to clients and caregivers in the home environment in order to maintain optimal quality of life and independence. Services include:
- providing information and support to the patient, family and friends
- assisting with plans for future care and support
- providing support during grief and bereavement
- discussions of issues and options related to care
- helps with symptom management, such as pain, nausea and shortness of breath
- connections to services or agencies
- assess the need for special equipment and accessing certain benefits through the program.
Care may be provided at home, in a personal care home, long-term care facility or in the hospital.
Eligibility
Services are based on client need as approved on the Care Plan by the Case Manager.
