Service Description

Provides support to maintain housing.

Four Winds Housing Program:

  • Staff conducts home visits every week to assist with tasks and goals. Focuses on client’s ability to maintain stable tenancy and transition into independent living.

Sohkâyâwin Intensive Case Management Program:

  • Provides support for individuals with complex needs or those who have experience homelessness for long periods of time. Focuses on client’s ability to maintain stable tenancy and transition into independent living.

The Housing Support Team provides:

  • Transportation provided for the client to appointments and programs
  • Helps to set up appropriate support services in the mental and physical health sector
  • Assists with rent and bill payments, as well as budgeting
  • Helps connect the client to recreational, cultural and spiritual programming
  • Assists in matters of food security, home maintenance and daily living skills
  • Helps the client address addictions concerns and seek appropriate support
  • Navigates social assistance, Service Canada, legal courts, child protection, or any other government programs they may be connected to.
