Service Description
Provides support and assistance to adults with physical or intellectual disabilities living independently.
Eligibility
Adults with physical or intellectual disabilities referred by the Community Living Service Delivery (CLSD).
Application Process
Applications are not accepted from the community at large. Referrals must be made through the Community Living Service Delivery (CLSD), which is the single point of entry for families.
