Service Description
Provides support to individuals with intellectual disabilities, to assist in living independently in the community without support most of the time. Areas of support will be determined for each person based on their needs. Support may include life skills, budgeting, appointments, housing and more.
Eligibility
Individuals must to be able to: 1. Access their own transportation 2. Participate in the activities of skill development 3 Take medications independently 4. Possess a willingness to gain further independence 5. Accept the risk and responsibility of choice 6. Possess a desire to live with their current financial means 7. Handle emergency situations that arise independently.
