Service Description

Provides youth, aged 16 – 21, a long-term home in an apartment. Youth receive day-to-day support from a staff member, who guides them in their transition to independence outside of care and offers assistance in the areas of community connections, life skills, education, health and self advocacy. Referrals come from the Ministry of Social Services.

Eligibility

Youth 16 - 21 years of age, who are currently involved with the Ministry of Social Services.
