Service Description
Provides home based services and supports to families with children who are birth to school age, who are developmentally delayed or “at risk” for delay. Services identify and help implement realistic goals for their children and family.

Eligibility
Children birth to school age who are developmentally delayed or at risk of developmental delays and their families.
Application Process
Families can call the program directly to receive service, a referral is not necessary to access this program.
