Service Description

This is the Central Intake service.

Helps patients find solutions to health concerns. Staff helps clients:

  • sort out their needs
  • look at options/choices
  • connect with a variety of appropriate services
  • become as independent as possible.

Some of the designated client groupings include: Acquired Brain Injury, Palliative Care, Pediatrics, Geriatrics, HIV/AIDS, Quick Response Program (Emergency).

Provides access to and determines eligibility for:

  • Special Care Homes
  • Home Care
  • Community Therapies
  • Community Volunteer Services.

Provides information for:

  • Personal Care Homes
  • Community housing options
  • other resources.
