Service Description
This is the Central Intake service.
Helps patients find solutions to health concerns. Staff helps clients:
- sort out their needs
- look at options/choices
- connect with a variety of appropriate services
- become as independent as possible.
Some of the designated client groupings include: Acquired Brain Injury, Palliative Care, Pediatrics, Geriatrics, HIV/AIDS, Quick Response Program (Emergency).
Provides access to and determines eligibility for:
- Special Care Homes
- Home Care
- Community Therapies
- Community Volunteer Services.
Provides information for:
- Personal Care Homes
- Community housing options
- other resources.
