Service Description
Provides immediate access to adequate and affordable housing for people who have been chronically homeless, and then collaborates with them to address unmet physical and mental health issues, financial, social, and spiritual needs. Utilizes intensive case management.
Eligibility
Individuals who are experiencing frequent episodes of, and/or, chronic homelessness.
