Service Description

Provides support for people identified with mental health and addiction issues in the community. Services, which are short-term and intensive to support individuals while connecting them to longer-term services or until stabilization, include:

  • Support transitioning home from hospital or detox
  • Follow-up support as requested by psychiatry
  • Crisis intervention

Eligibility

Adults 18 years or older.
